Exploring Diabetic Patient Care Pathways in Peja

On 20 March 2024, a team of Swiss TPH projects in Kosovo facilitated a visit to the Main Family Medicine Center and General Hospital in Peja for renowned Slovenian diabetes expert, Samo Fakin. The visit is part of the Integrated Health Services (IHS) activities supporting the operationalization of Non-Communicable Disease (NCD) care pathways between selected General Hospitals (GHs) and Primary Healthcare (PHC) facilities.

Mr. Fakin’s visit was aimed at gaining insights into the care pathways, as well as assessing the clinical pathways between primary and secondary healthcare levels for patients with diabetes. He shared his expertise and experience in topics like empowering patients with diabetes, improving the healthcare level, the importance of effective treatment methodologies, and patient education in managing diabetes. He also addressed queries from healthcare professionals of both levels.

The activity underscores the commitment to improve NCD care pathways and ultimately enhance the well-being of diabetic patients in the region.
