Pjes\u00ebmarr\u00ebsit ndan\u00eb informata nga terreni, hart\u00ebzuan sfidat ekzistuese dhe bashk\u00ebprojektuan veprime t\u00eb cilat b\u00ebjn\u00eb q\u00eb z\u00ebrat e pacient\u00ebve t\u00eb d\u00ebgjohen m\u00eb shum\u00eb n\u00eb planifikimin e ardhsh\u00ebm sh\u00ebndet\u00ebsor.<\/p><p class=\"p1\">Pun\u00ebtoria n\u00ebnvizoi rolin e OSHC-ve n\u00eb krijimin e lidhjeve midis komuniteteve dhe institucioneve, duke vepruar si kanale t\u00eb besueshme p\u00ebrmes t\u00eb cilave shprehen nevojat dhe t\u00eb drejtat e pacient\u00ebve.
