Vizita \u00ebsht\u00eb pjes\u00eb e aktiviteteve t\u00eb projektit Sh\u00ebrbime Sh\u00ebndet\u00ebsore t\u00eb Integruara (IHS) q\u00eb mb\u00ebshtesin funksionalizimin e rrug\u00ebve t\u00eb kujdesit p\u00ebr s\u00ebmundjet kronike jongjit\u00ebse (SJN) n\u00eb Spitalet e P\u00ebrgjithshme (SP) dhe institucionet e Kujdesit Par\u00ebsor Sh\u00ebndet\u00ebsor (KPSH) t\u00eb p\u00ebrzgjedhura.<\/span><\/p><p><span style=\"font-weight: 400;\">Vizita e Z. Fakin kishte p\u00ebr q\u00ebllim marrjen e njohurive mbi shtigjet e kujdesit, si dhe vler\u00ebsimin e rrug\u00ebve klinike midis niveleve t\u00eb kujdesit par\u00ebsor dhe atij dyt\u00ebsor sh\u00ebndet\u00ebsor p\u00ebr pacient\u00ebt me diabet. Ai ndau ekspertiz\u00ebn dhe p\u00ebrvoj\u00ebn e tij n\u00eb tema si fuqizimi i pacient\u00ebve me diabet, p\u00ebrmir\u00ebsimi i nivelit t\u00eb kujdesit sh\u00ebndet\u00ebsor, r\u00ebnd\u00ebsia e metodologjive efektive t\u00eb trajtimit dhe edukimi i pacient\u00ebve n\u00eb menaxhimin e diabetit.
